Once you’ve called us and scheduled your new patient appointment, then please complete your new patient forms before your appointment. These forms can be filled out from your computer. Once you hit “Submit” it will automatically be sent to our office and populate the account we created for you. If you would rather print the forms, please click on the Printable PDF. Your First Experience
When you call our office for the first time, you will be greeted by one of our Patient Care Coordinators, Annette, Diane, or Nicole, who will gather your contact, dental health, and insurance information before scheduling your first appointment.
Your first appointment is a new patient or initial exam which includes:
- A full mouth series of x-rays and
- A comprehensive oral exam with Dr. Alan Ing or Dr. Yunsang Park
Any follow-up appointments may include:
- A professional cleaning with Nora, Didi, Elana, or Rhonda, our Registered Dental Hygienists
- Treatment with Dr. Ing or Dr. Park
Chelsie, Maricar, Jess or Skylar, our dental assistants, will be by your side throughout your visits with Dr. Ing or Dr. Park. They assist the doctor to gather and record the information needed to assess and determine the health of your teeth. They will take intraoral photos, digital x-rays of your teeth, keep your mouth clean and dry during procedures, and if necessary, take impressions of your teeth for bridges, crowns, dentures and other treatment.
A comprehensive examination helps our doctor to determine if there is an immediate problem that needs to be fixed, such as a cavity. During this visit, Dr. Ing or Dr. Park will learn about the smile you would like to achieve. Based on your wishes, our doctor will present a treatment plan to restore any problems, and offer you elective cosmetic options to achieve the smile you’ve always wanted. The treatment that you receive is ultimately up to you.
Following your initial examination, we will then schedule your next appointment with one of our hygienists who will provide a professional cleaning of your teeth and assess the health of your gums. Cleanings are usually performed twice a year, although your periodontal condition will determine the recommended number and type of cleanings per year.
